Most people take semaglutide to lose weight or to manage type 2 diabetes, and the evidence that it does both is strong. But a digest of the trial numbers misses half the story, so this page covers the other half plainly: what people who use it actually describe, and who has a real reason to be careful.
The pattern in patient reports is consistent. The biggest upside people name is that the constant mental chatter about food — what some call "food noise" — goes quiet, often in the first week or two. The most common downside is nausea, which tends to peak early and after each dose increase. Below, the reported effects come first (clearly labeled as anecdotes, not proof), then the safety cautions that are grounded in the clinical-trial and pharmacovigilance record and cited to their studies. The benefits people name most
- Quieter "food noise" and faster fullness (frequently reported). By far the most common benefit is that the background chatter about food goes quiet, often within the first week or two. People say they feel full faster, eat a third to a half of their old portions, and stop obsessing over the next meal — many call it the single most life-changing effect.
- Cravings drop (frequently reported). Sweet-tooth and sugar cravings fall sharply or vanish, and fried, greasy, high-fat foods stop appealing, sometimes turning slightly off-putting. Several people describe naturally drifting toward fruit, vegetables and lighter meals, often within the first few weeks.
- Weight loss (frequently reported). The large majority of reviewers report losing weight, often describing it as steady and substantial over several months, with the pace slowing after the early period. Many tie it directly to eating much less rather than to any change in exercise.
- Better blood-sugar control (commonly reported). Among people using it for type 2 diabetes, a common theme is markedly improved blood-sugar and A1C readings, with some describing fasting numbers and long-term averages dropping into normal ranges, plus steadier daytime energy.
- Less desire to drink alcohol (occasionally reported). A recurring secondary observation is that the urge to drink fades along with food cravings; some people simply lose interest in alcohol. This is widely discussed in patient communities as an unexpected side benefit.
The downsides people name most
- Nausea, sometimes with vomiting (frequently reported). Nausea is the single most-reported side effect, mentioned by roughly a third of reviewers, with a subset escalating to vomiting at its worst. It tends to peak in the first weeks and after each dose increase, often easing within a week or two, and flares after overeating or fatty food. Many manage it with smaller, lighter meals and plenty of water.
- Sulfur or "egg" burps (commonly reported). A distinctive complaint is foul-smelling burps compared to rotten eggs or sulfur, often arriving after a dose increase and sometimes lasting hours to weeks, frequently with bloating and a sense of food sitting too long. People describe them as embarrassing and note they show up far more often than official lists suggest; some find they fade with time.
- Bowel changes — constipation and diarrhea (commonly reported). Disrupted bowel habits are among the most-mentioned digestive complaints, sometimes alternating between extremes. Constipation shows up as hard, infrequent stools; diarrhea is worse in the days right after a dose or after rich food. A few note the constipation is partly just from eating so little.
- Acid reflux and heartburn (occasionally reported), often alongside burping and bloating and tracking with dose increases; some manage it with over-the-counter acid reducers.
- Fatigue early on (commonly reported), especially in the first day or two after each injection and during the early weeks, usually easing with time.

Semaglutide side effects and safety cautions
These semaglutide side effects and cautions are grounded in the clinical-trial and pharmacovigilance record and cited to their sources. They describe documented signals and label warnings — not instructions, and not a personal risk assessment.
Gastrointestinal intolerance, especially during dose escalation. Nausea, vomiting, diarrhea and constipation are the dominant adverse effects in trials and the leading cause of stopping. A pooled analysis of the STEP weight-management program found these events were predominantly mild-to-moderate and transient, concentrated around the titration period [19]; a dedicated safety review reported nausea in roughly one-third of patients [7], and real-world pharmacovigilance reporting is likewise dominated by gastrointestinal events [20]. This is clinical, not theoretical: the slowing of stomach emptying that drives the GI effects is part of how the drug works.
Personal or family history of medullary thyroid carcinoma or MEN-2 (boxed warning). GLP-1 receptor agonists carry a boxed warning for thyroid C-cell tumors derived from rodent studies, in which such tumors occurred at supratherapeutic exposures. A dedicated assessment of thyroid risk concluded that available human data do not establish a clear increase in thyroid cancer attributable to semaglutide, so the signal is best framed as unconfirmed in humans [21]; even so, a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2 is treated as a contraindication on the strength of the rodent finding [7].
Acute pancreatitis (class warning). Acute pancreatitis is a recognized class warning for GLP-1 receptor agonists, and treatment is conventionally stopped if pancreatitis is suspected. A dedicated safety review notes that pancreatic-cancer signals remain ones for which definitive conclusions cannot yet be drawn owing to low incidence, rather than confirmed associations [7]. The caution is precautionary, not a demonstrated quantitative risk.
Gallbladder and biliary disease. A dedicated safety review found an increased risk of biliary disease (gallstones) with semaglutide, attributed largely to the rate and magnitude of weight loss rather than direct drug toxicity — but the increase versus placebo is a real trial and pharmacovigilance finding, not merely theoretical [7].
Pre-existing diabetic retinopathy with rapid glycemic correction. In SUSTAIN-6, rates of diabetic-retinopathy complications were significantly higher with semaglutide (HR 1.76; 95% CI 1.11-2.78), concentrated among patients with pre-existing retinopathy undergoing rapid lowering of HbA1c [4]. The leading interpretation is early worsening driven by the speed of glycemic correction rather than direct retinal toxicity; monitoring is advised when blood sugar is corrected fast [7]. This is a trial signal in people with diabetes.
Loss of lean (muscle) mass. A STEP-program body-composition substudy reported that the weight lost with semaglutide comprised both fat and a meaningful proportion of lean mass [22]. Because rapid, large-magnitude weight loss can erode muscle, this raises a sarcopenia concern (particularly in older adults) and has driven research into protein intake and resistance training to preserve lean tissue. The lean-mass loss is an observed trial finding; the downstream sarcopenia risk is a mechanistically reasoned, extrapolated concern.
Weight regain after stopping. Discontinuation is followed by substantial regain. In the STEP 1 trial extension, participants regained a mean of roughly 11.6 percentage points of body weight within one year of stopping, and cardiometabolic improvements reverted toward baseline [23]; the STEP 4 randomized-withdrawal design likewise showed regain after switching to placebo [24]. This frames obesity pharmacotherapy as a chronic rather than curative intervention.
Semaglutide hair loss
Hair loss appears in the record as a pharmacovigilance signal, not a confirmed drug toxicity. A disproportionality analysis identified a reporting signal for alopecia associated with semaglutide and tirzepatide [25], and a separate retrospective dermatology study linked telogen effluvium — a reversible, diffuse shedding — to the magnitude and rate of weight loss [26]. Taken together, the signal is most consistent with rapid-weight-loss-associated telogen effluvium rather than a direct effect of the molecule, and it is the same mechanism people describe anecdotally above. The shedding is generally reversible. This is pharmacovigilance-level evidence with a weight-loss-driven mechanistic reading, not a quantified risk.
Then and now
Semaglutide is the product of decades of incretin-peptide chemistry, built on an earlier GLP-1 analogue and engineered for once-weekly dosing through DPP-4 resistance and albumin-binding fatty-acid acylation. It first reached FDA approval for type 2 diabetes in 2017, with an oral once-daily formulation following in 2019-2020 and a chronic weight-management indication in 2021 [2]. Its cardiovascular-outcomes evidence read out in SELECT in 2023 [3] and its kidney-outcomes evidence in FLOW in 2024 [5], with a metabolic dysfunction-associated steatohepatitis (MASH) indication following in 2025 [8]. During a federally declared shortage from roughly 2022 to early 2025, compounding pharmacies were permitted to produce semaglutide [4]; that pathway was curtailed once the shortage was declared resolved. It is, today, a fully approved medicine with one of the deepest outcome-trial records in its class.
